Madina does it with a portable machine with test strips. One pack is enough for two months. Normal blood values are 2.5-3.5 units. Any deviations from these figures require adjustment of the dose of the drug. It is dangerous both to reduce it unreasonably - because of clots it will break the heart valve, and to increase it - it can open bleeding.
